Leaves of Absence
A leave of absence is defined as an unpaid excused absence from work where the employee has received prior approval from the employee’s manager. Requests for leave of absence must be submitted to the employee’s manager and Human Resources (HR@sonsray.com) in writing at least 30 days prior to the date the leave is to begin. In cases of unexpected emergency, the request should be submitted as soon as reasonably possible. In the case of a request for a medical leave of absence, a statement from a licensed health care provider giving the date of the onset of the disability and the expected date of return to duty will be required. This pertains to all medical disabilities, whether the cause is job related or non-job related. In the event of an unexpected medical emergency, the employee should make arrangements for someone to notify the employee’s manager immediately. Employees must comply with the Company’s normal call-in procedures, as applicable, during any leave of absence.

How to Submit a Leave Request
- Submit an LOA Request Form to hr@sonsray.com
- Submit a requestto AbsencePro (third party for managing leaves).
- Submit certification documents to AbsencePro upon their request.
What to Know
- Intermittent leaves: Eligible leaves do not need to be taken all at once. Please talk to HR and your manager to agree upon any schedule changes.
- Please coordinate health insurance premium payments with HR. For leaves extending beyond a 12-week consecutive timeframe, continuance of insurance will be through COBRA.
- Sonsray defines the 12-week period as a “12-month period measured forward from the first date an employee takes FMLA leave”.
- Using available sick and/or vacation time during leave is optional.

My HSA Benefits
This process must be completed immediately upon enrollment
- Log in to your Paylocity Self Service Portal
- Click on Spending Accounts
- Read and Accept the Custodial Agreement, Electronic Disclosure, and Patriot Act Requirements.
- Click on the Account Settings Tab at the top of the screen.
- Enter Beneficiary information.
- Review/add Dependent(s)
- Enter personal bank account information for Online Account Transfers.
- Primary method of payment is Debit Card.
- Alternate Method of Payment will be the bank account identified as your Main Account.
- Access HSA Forms in the Self Service Portal.
- Click Spending Accounts.
- Select Knowledge Base from the Help drop down menu.
- Search HSA Resources for Paylocity FLEX Services.
PAYROLL: After you sign up, you will get an authorization form from HR, that requires your signature. This authorization form will your employer to make deductions per payroll so that they can deposit that money into your HSA account going forward.

All full-time employees who work at least thirty (30) hours per week are eligible for Voluntary Life coverage. Your coverage becomes effective on the first day of the month coinciding with your date of hire. Eligibility:
All full-time employees who work at least thirty (30) hours per week are eligible for coverage. Your coverage becomes effective on the first day of the month coinciding with your date of hire. If you do not enroll for benefits within 31 days of becoming eligible, you will have to wait until the next open enrollment.
My Retirement Benefits
Eligibility:
After 60 days of employment, you will be eligible to enroll in the 401k plan, with enrollment taking effect only at the start of the following quarter. To decline automatic enrollment, please contact Fidelity at (800) 343-3548 before you reach your 60-day employment mark.
New Hire Automatic Enrollment
To get your retirement savings off to a good start, Sonsray will automatically enroll you in the 401(k) plan at a contribution rate of 7% of your annual salary. If you want to opt out of the automatic enrollment, please contact Human Resources.
You may make your contributions using one or both of these plan options:
- Traditional 401(k) Plan: If you choose this option, you will fund your account with before-tax contributions. That means your contributions will be deducted from your paycheck before taxes are taken out. You will not have to pay taxes on this money or on your account’s investment earnings until you withdraw your funds at retirement.
- Roth 401(k) Plan: If you choose this option, your contributions will be deducted from your paycheck after taxes are taken out, so you will not get the immediate tax savings you get with the traditional 401(k) plan. However, when you take your money out at retirement, it will be tax-free, this includes both your original contributions and the investment earnings that may have accumulated over the years.
Your Savings
You may save any whole percentage of your annual salary in the plan, up to the maximum contribution allowed by the Internal Revenue Service. For the 2025 calendar year, the maximum contribution is $23,500. Participants age 50 or older are allowed to make an additional “catch-up” contribution of up to $7,500.
Sonsray Contributions
The Company may also make discretionary contributions (after one year of service), which may be increased, decreased, or eliminated. All contributions to the retirement plan are subject to limitations imposed by the Internal Revenue Code and the terms of the Company’s 401(k) plan and applicable policies adopted to govern such plans.
